Haven Trust receives referrals from Housing Options, WCADA, Drug Aid, The Bays Project, The Wallich, Shoreline, Social Services, The Probation Service, Solicitors acting on behalf of prisoners due for release, The CARAT Team and The Prison Chaplaincy, as well as CJIT Swansea (Criminal Justice Intervention Team).
In exceptional circumstances self referrals will initially be accepted but must be processed as well by Housing Options to ascertain the candidate’s housing history and to complete a risk assessment.
Haven Trust will endeavour to send a Referral and Risk Assessment Form by return of post or at least within seven days.
A Risk assessment/Referral Form will need to be completed by the referring agency.
On receipt of a completed Referral and Risk Assessment Form Haven Trust will work to effect a decision within seven days.
Selection of service users is by way of interview. This will include the need to complete a needs analysis form with corresponding targets. This will then lead to a support plan being mutually drawn up and implemented.
No one is allowed admittance if they are taking unprescribed drugs or are drinking alcohol or under its effects.
The service user will have read, understood and agreed to abide by the house rules and procedures, which will be attached to the Licence/Tenancy which the service user will need to sign to take up residency.
